Clinical Aspect of Hepatic Coma and Its Problem for Nursing Care


Abstract
Hepatic coma is not rare seen in Korea due to increase of tendency of acute and chronic hepatopathy which it may be developed from to take a her¢¥s medicine and hepatotoxic drugs recently. For 3 years, from 1968 to 1970. 30 cases of hepatic coma hospitalized in Pep. of Int. Med. Chung nam Medical Center were clinically and nurselly studied.
a. Radio of developing of hepatic coma among the admitted patients of G-1 cases was about 13%.
b. In sex radio, the man and female is 3 : 1.
c. The underlying disease of the hepatic coma included cirrhosis of the liver 23 patients(76. 7%, hepatoma 2 patients (6.7%), drug induced hepatitis 2 patients (6.7%), fluminating hepatitis 2 patients (6.7%), acute hepatitis 1 patient (3.3%), The precipitaing factors of hepatic coma were 17 cases of upper G-I bleeding, abdominal synthesis 2 cases, infection 2 cases, 2 cases of sedation and analgesic among the 23 cases or chronic hepatopathy.
d. There were, fever history more than 38¡ÆC in half cases and leucocytosis (more than 10, 000mm3) and shift to in 60% each. Urine albumin was detected in all cases except 2 cases.
e. Liver function tests revealed diffuse hepatocellur damage but it may be helpful to diagnosis that acute elevation of serum bilirubin, lowering of serum cholesterol, marked prolongation of prothrombin time.
f. Mortality was very high but may depend upon underling disease and the causes of precipitating factors.
